Dear HAE friends,
Among the thousands of published HAE-related studies, there are none that provide an adequate description of how HAE affects patient's lives. As a result, the US HAE Association cordially invites you to participate in our anonymous, web-based study that will collect information on HAE's financial, economic, and social/personal burden. The data collected for this study will be used to help physicians design better HAE treatment plans, and provide health insurance companies with information they need to make informed decisions regarding covering and reimbursing HAE therapies.
This study does not involve the use of any drugs or medical interventions; it is only an anonymous web-survey that will probably take about 30-60 minutes to fill out. After you have completed the survey once and submitted all of your responses, please do not return to the survey to take it again.

You will then be asked to provide informed or medical consent electronically, and after doing so you can start filling out the survey. You are not obligated to complete the survey once you begin filling out the questionnaire. Regulations require that we limit the survey to patients age 19 or older in the states of Alabama and Nebraska or 18 and older elsewhere.
